Your purchase order should contain the following information:

•	Your name, telephone number and fax number
•	Full invoicing and shipping addresses
•	Purchase order number (for tracking purposes)
•	Affymetrix part number
•	Quantity
•	Your pricing
•	In Europe, please include your VAT number
•	Any special requirements

STANDARD ORDER TERMS

Our U.S./Canadian standard order terms are the following:

•	Freight & Handling are prepaid and added to the invoice
•	Payment terms are Net 30
•	Freight on Board (F.O.B.) Origin

Our European standard order terms are the following:

•	Delivered, duty paid excluding value added tax on imports
•	Payment terms are Net 30
