To join the Affymetrix Core Lab Program your lab must:
	•	Be part of a non-profit organization
	•	Have the ability to successfully provide microarray analysis service to the institution
	•	Be qualified by an Affymetrix local sales representative
	•	Be approved by an Affymetrix sales manager
	•	Currently possess an Affymetrix GeneChip® System
	•	Have a valid instrument service contract at Elite or Assurance level

To purchase a subscription to the Affymetrix Core Lab Program, please send the following to Affymetrix Customer Service:
	•	A completed application form
	•	A purchase order for the Bronze Core Lab Program
	•	Applications will not be processed without a purchase order
